CASE MANAGEMENT SERVICES

The vulnerable populations service offers Case Management of clients with a greater intensity of needs than standard settlement services. CFN uses wraparound and ecological approaches to ensure newcomers’ entire life circumstances are taken into consideration when creating solutions with them.

 

The Case Manager will work with newcomers to help explore options and then develop and implement a service plan of action. The service offers various assessments, mental health settlement services, health settlement services, referral to any required services, direct support, groups supports, and monitoring and follow-up over time to ensure settlement success.
